5 ways to make sure you are not vitamin D deficient

Many of us know how important vitamin D levels are to our overall health and that we get most of our intake through outdoor summer sun UVB exposure. But what can we do in the winter when outdoor UVB is unavailable at our Canadian latitude? Here are some practical ways to make sure you are not vitamin D deficient this winter:

1. Test your vitamin D level– The only way to know if you are vitamin D deficient is through a 25(OH)D blood test. This can be prescribed by your doctor and there may be a fee involved. Another option would be to purchase a home blood test kit through an organization like GrassrootsHealth. Either way, make sure you get your test score and compare it to the level recommended by over 50 vitamin D scientists of 100-150 nmol/L. If you are not within this ideal level, adjust your intake either up or down, using some of the options below, and re-test again after 3 months since it takes some time to bring the levels up.

2. Take a trip south – Approximately 90% of our vitamin D is produced in our skin with UVB light from the sun. There is nothing better than getting away to a sunny destination and soaking up some rays. To produce vitamin D in your skin the location needs to be below 35° N latitude or roughly below Atlanta, GA. Vitamin D is best produced at midday when the sun is high in the sky with the UV index above 3 and your shadow shorter than your height. Consider acclimatizing your skin before going south so you don’t burn as easily. Remember, sunscreen applied properly reduces your vitamin D production by up to 95+%. Try getting some sun exposure for vitamin D first and then applying sunscreen to make sure you do not burn.

3. Use a sunbed – Artificially produced UVB can be a surrogate for sunlight in winter. Make sure you use a facility that has a trained operator controlling the equipment. A study I co-authored in 2010 showed that indoor tanners have some of the highest vitamin D levels in Canada. A photon of light is a photon of light whether from the sun or produced in a sunbed. For people with malabsorption issues, such as those with inflammatory bowel disease (IBD), who cannot absorb vitamin D from food or supplements, this would be their only option in the winter. Remember to use protective goggles and consider reducing the suggested session time by 50% and have at least 48 hours between sessions.  There is an increased risk of skin cancer from UV overexposure whether from outdoor sun or sunbed exposure and caution needs to be taken. Know your skin, and skin type. Do not burn!

4. Supplement with Vitamin D3– Supplements throughout the winter are the next best option to UVB exposure. However, understand that vitamin D supplements do not provide all the beneficial photo products that the sun or UV produces. The key with vitamin D3 supplements is that you need to take enough to be comparable to what you would make in the summer sun. For most adults that would be up to 4,000 IU per day. Vitamin D3 supplements are safe, easy to find and inexpensive.

5. Eat more vitamin D rich foods – Food sources overall do not contain much vitamin D but can help contribute a small amount to your 4,000 IU daily intake. Oily fish such as wild salmon are one of the best food sources and contain up to 500 IU per serving. Milk is fortified and contains 100 IU/cup. Other sources to consider include mushrooms and egg yolks.

    Vitamin D is an 'inexpensive solution' to drugs as scientists discover the sunshine supplement repairs and prevents damage to the heart caused by diabetes and high blood pressure.

    The sunshine supplement stimulates the production of nitric acid, which is involved in regulating blood flow and preventing the formation of blood clots, according to the first study of its kind.

    It also reduces 'internal stress' in the cardiovascular system, which could avoid heart-related incidents, the research adds.

    Study author Dr Tadeusz Malinski from Ohio University, said: 'There are not many, if any, known systems which can be used to restore cardiovascular cells which are already damaged, and vitamin D can do it.

    'This is a very inexpensive solution to repair the cardiovascular system. We don't have to develop a new drug. We already have it.'

    Heart disease, which is any disorder affecting the organ or blood vessels, is the leading cause of death in adults in the US, resulting in one in four fatalities.

    How the research was carried out 

    The researchers analyzed heart cells taken from a group of Caucasian and African Americans.

    They used special sensors to track the impact of vitamin D in these cells.

    'This is a very inexpensive solution'  

    Dr Malinski said: 'Generally, vitamin D is associated with the bones. However, in recent years, in clinical settings people recognize that many patients who have a heart attack will have a deficiency.

    'It doesn't mean the deficiency caused the heart attack, but it increased the risk.

    'There are not many, if any, known systems which can be used to restore cardiovascular cells which are already damaged, and vitamin D can do it.

    This is a very inexpensive solution to repair the cardiovascular system. We don't have to develop a new drug. We already have it.'

    The researchers believe vitamin D may also be able to repair damage to other types of cells, such as those found in the brain after a stroke.

    The findings were published in the journal Nanomedicine.

    Paper Reiterates UV’s Link to ‘Overall Good Health’

    The cardiovascular impact of sunlight — reducing blood pressure by triggering the release of nitric oxide from the skin — outweighs any potential risk factors of exposure, researcher Dr. Richard Weller wrote in a 2016 paper published in the journal Blood Purification. Weller pioneered research on the subject in 2013.

    Here are some of the key statements from the new paper:

    “All-cause mortality should be the primary determinant of public health messages. Sunlight is a risk factor for skin cancer, but sun avoidance may carry more of a cost than benefit for overall good health.”

    “For the general white-skinned population, sunlight is the major preventable risk factor for skin cancers, but the pattern of sun exposure varies with cancer type. Intermit- tent sun exposure and sunburn, particularly in childhood, increase the risk of melanoma, whereas chronic occupational exposure may be protective”

    “Dose dependently, the higher the sun-seeking behaviour, the lower the all-cause mortality, with those scoring 4 having half the mortality of sun- avoiders. Extrapolating from these data, the authors calculate that 3% of deaths in Sweden can be accounted for by inadequate sun.”

    “Importantly, vitamin D is not solely responsible for these proposed health benefits of sunshine. Supplementation with oral vitamin D is not adequate to reduce cardiovascular disease. Alternative mechanisms must exist to account for these benefits of sunlight.”

    “Ultraviolet therapy might well have a therapeutic role beyond the treatment of skin disease.”

    “Public health advice on sunlight exposure is at the crossroads. Almost a century of data has confirmed the carcinogenic effects of UV radiation on the skin, and delineated the mechanisms by which this occurs. There is however a remarkable absence of any evidence that UV reduces lifespan, in sharp contrast to other risk factors (e.g. hypertension, smoking, alcohol) on which we advise. A substantial body of evidence shows that sunlight has health benefits and that these are independent of vitamin D and thus cannot be reproduced by oral supplementation. The UV-induced reduction of cutaneous nitrate and its export to the systemic vasculature, which I have helped delineate, is an additional mechanism by which sunlight may exert beneficial effects on health, but other mechanisms surely exist. All-cause mortality and its reduction should be the primary aim of physicians, not the narrow avoidance of skin cancer.”

    WASHINGTON – Millions of U.S. children have disturbingly low Vitamin D levels, possibly increasing their risk for bone problems, heart disease, diabetes and other ailments, according to two new studies that provide the first national assessment of the crucial nutrient in young Americans.

    About 9 percent of those ages 1 through 21 – about 7.6 million children, adolescents and young adults – have Vitamin D levels so low they could be considered deficient, while an additional 61 percent – 50.8 million – have higher levels, but still low enough to be insufficient, according to the analysis of federal data being released Monday.

    “It’s astounding,” said Michal L. Melamed of the Albert Einstein College of Medicine in New York, who helped conduct one of the studies published online by the journal Pediatrics. “At first, we couldn’t believe the numbers. I think it’s very worrisome.”

    Low Vitamin D levels are especially common among girls, adolescents and people with darker skin, according to the analysis of a nationally representative sample of more than 6,000 children. For example, 59 percent of African American teenage girls were Vitamin D deficient, Melamed’s study found.

    The researchers and others blamed the low levels on a combination of factors, including children spending more time watching television and playing video games instead of going outside, covering up and using sunscreen when they do go outdoors , and drinking more soda and other beverages instead of consuming milk and other foods fortified with Vitamin D.

    “This appears to be another result of our unhealthy lifestyles, including a sedentary society that doesn’t go out in the sun much,” Melamed said.

    The analysis and an accompanying federal study also found an association between low Vitamin D levels and increased risk for high blood pressure, high blood sugar, and a condition that increases the risk for heart disease and diabetes, known as the metabolic syndrome.

    Taken together, the studies provide new evidence that low Vitamin D levels may be putting a generation of children at increased risk for heart disease and diabetes, two of the nation’s biggest health problems that are also increased by the childhood obesity epidemic.

    “These are very important studies,” said Richard S. Rivlin, a past president of the American Society for Clinical Nutrition who was not involved in the research. “They show the number of people who have high rates of Vitamin D deficiency is really very frightening.” Other researchers urged caution.

    “The bottom line is that these numbers are interesting,” said Frank R. Greer of the University of Wisconsin at Madison, who served on a panel that recently doubled the American Academy of Pediatrics recommendations for daily Vitamin D intake. “But I’m not ready to make a great hue and cry until we have more data. I think we should use them for further research to determine their significance.”

    The findings come as the National Academy of Sciences Institute of Medicine is reviewing the federal government’s official guidelines for recommended daily intake of Vitamin D. A public hearing is scheduled as part of that process at the National Academy of Sciences in Washington on Tuesday.

    Bowlegs  — The most well-known health problem associated with Vitamin D deficiency is rickets, which can cause bowlegs and other bone malformations. Milk was fortified with Vitamin D in the 1930s to eliminate the disorder. But during the 1990s, doctors in several cities reported unusual numbers of cases, primarily in babies being breast-fed and mostly among African American children. A number of studies began to indicate that deficiencies might be common among adults – small studies found the same among children – and that deficiencies may be associated with a host of health problems. But the new studies are the first to examine the issue nationwide in young people.

    Those who had low Vitamin D levels were more likely to have lower levels=20 of a hormone associated with healthy bones, higher blood pressure levels and lower levels of “good” cholesterol, which helps protect against heart disease, Melamed and her colleagues found.

    In the second study, Jared P. Reis of the National Heart, Lung and Blood Institute analyze d data from about 3,500 adolescents from surveys conducted between 2001 and 2004, and found that compared with those with higher Vitamin D levels, those with the lowest levels had more than double the risk of having high blood pressure and blood sugar and about four times the risk for the metabolic syndrome, a group of symptoms that increases the risk for heart disease and diabetes.

    Some longtime proponents of the health benefits of Vitamin D seized on the findings to urge parents to ask doctors to test their children’s Vitamin D levels, consider increasing supplementation or make sure children spend more time outdoors to boost their levels.

    “The sun has been demonized for years and as a result, people have avoided any direct exposure to sunlight,” said Michael F. Holick of the Boston University School of Medicine. “I think that’s the wrong message.”

    But others said they worried that encouraging children to spend more time in the sun could lead to more skin cancer, which is already the leading type of cancer.

    UVASUN’s Response:
    As many of you may already have heard, there has been a recent bashing of tanning beds in the media as of late due to a new WHO classification. We would like to clarify much of the misleading hype and put things into perspective.

    “…International cancer experts have moved tanning beds and other sources of ultraviolet radiation into the top cancer risk category, deeming them as deadly as arsenic and mustard gas…”

    It’s absolutely preposterous to make the comparison between sun beds and hazardous chemicals such as arsenic and mustard gas. The sun itself has been included in the top risk category since 1992, so it is no surprise that tanning beds that emit the same UV light would be included in this category. The WHO (World Health Organization) expects the world population to be naive enough to believe that the single most important source of human life (the sun) is as deadly as poisonous gas.

    “…A new analysis of about 20 studies concludes the risk of skin cancer jumps by 75 percent when people start using tanning beds before age 30…”

    The study’s authors admit they did not adjust for outside sun exposure or sun sensitivity due to skin type. Even if the information is accurate, let’s examine the 75% risk increase and what it means with this example – Your relative risk of getting a fatal form or skin cancer is 0.12%. If that number increases by 75%, then your relative risk is still only 0.21%. So scare tactics are used to make you believe that 75% of tanners will develop skin cancer. From the above example you can see THIS SIMPLY IS NOT TRUE.

    Also, most of the studies referenced above were conducted in the UK where a much greater percentage of the tanning population were skin type I. Skin type I burns very easily and very quickly and does not tan. Uvasun believes that those with skin type I should not be exposed to UVR. When skin type I was factored out of these studies, there is no evidence of an increased risk to skin cancer.

    Additionally, of the 23 studies (some dating back to the 1980’s) 5 were excluded for unusable data, 6 had results that suggested tanning beds actually REDUCE the risk of cancer, and 16 had results within the margin of error.

    “…Experts also found that all types of ultraviolet radiation caused worrying mutations in mice, proof the radiation is carcinogenic. Previously, only one type of ultraviolet radiation was thought to be lethal…”

    This simply is not true, there have only been “casual” correlations between UVR and skin cancer. This is the same as saying we should avoid water altogether because every single fatal drowning has included water.

    Additionally, the skin type of a lab mouse is significantly different than that of a human with skin type II-VI. At UVASUN, we believe that moderation is key and skin type I should not be exposed to UVR. Also, there is no reference to the amount or intensity of UVR that the mice were exposed to, this seems to be an intentional omission.

    “…The new classification means tanning beds and other sources of ultraviolet radiation are definite causes of cancer, alongside tobacco, the hepatitis B virus and chimney sweeping, among others…”

    The “others” that are included in this category are red wine, beer, and fish. This is another intentional omission considering the WHO has also conducted studies that conclude red wine in moderation is good for hearth health and fish in moderation can help lower bad cholesterol. Is it not absurd to say “a glass of merlot is as lethal as mustard gas”?

    The key word here is moderation. Moderate UVR directly stimulates the production of Vitamin D which is necessary for our health and has been shown to fight many forms of internal cancer. Therefore it is also absurd to conclude that “moderate exposure to UV is as lethal as arsenic”.

    “…But as use of tanning beds has increased among people under 30, doctors have seen a parallel rise in the numbers of young people with skin cancer. In Britain, melanoma, the deadliest kind of skin cancer, is now the leading cancer diagnosed in women in their 20s. Normally, skin cancer rates are highest in people over 75…”

    Since 1974 the number of dermatologists offices has nearly doubled from 5,700 to 10,700 by 2006 in the US. This is four times greater than the increase in US population during that same time period. Undoubtedly, many more Americans are visiting dermatologists and the screening of skin cancer has greatly improved thus leading to more detections.

    An interesting correlation to this data is the increased usage of sunscreens. During this same time period, the sunscreen industry grew from just a few million dollars in revenue per year to $35 billion. So using the same logic can’t we then say that there is a definite correlation between the increased use of sunscreen and the increase in skin cancer detections?

    You SHOULD Have a Sunbed Session

    The message from doctors has long been unequivocal – there is no such thing as a safe suntan because sun exposure causes skin cancer. However, despite being a cancer expert myself, I have just spent the past four weeks on holiday in South Africa, where I made sure I got out into the sun every day. Current medical advice is to cover up in the sun, but I believe the health benefits of exposure to its UVA and UVB rays greatly outweigh the disadvantages, even if that means using a sunbed during winter months. This is because the body converts sunshine into Vitamin D, a substance that helps build a healthy immune system and strong bones, and may even fight off depression and cancer.

    Vitamin D is known to play a vital role in metabolism of calcium, which is needed for strong bones. And last week a new study revealed it may help stave off Alzheimer’s Disease. Research on English adults aged 65 and over showed that as their Vitamin D levels decreased, mental impairment increased. Deficiency can also cause osteomalacia – a weakening of the bones that may cause rheumatism or chronic backache – and children can develop rickets, a condition in which the bones do not develop properly.

    Research has shown that without it we are more prone to everyday infections such as colds and flu, and also more worrying ones, such as tuberculosis (TB). But perhaps more surprisingly, there is also evidence that Vitamin D could protect us from cancer. Recent studies have found those countries with the most annual sunshine have the fewest cases of colon, prostate and breast cancer (although exactly what role Vitamin D plays isn’t totally clear in these instances). In one case, sunshine exposure was linked to a 40 per cent reduced risk of prostate cancer. And research published in the American Journal Of Clinical Nutrition in 2007 reported a possible reduction of breast cancer in women who took Vitamin D tablets.

    The idea that Vitamin D is essential for good health is not new; nor is the connection to sunlight. In Britain just after the turn of the 19th Century, children in cities had less exposure to sunlight and became crippled by rickets. It was also discovered that TB – which was commonplace – could be treated with cod liver oil, another good source of Vitamin D, and sunlamps – early versions of sunbeds that provide artificial UV light similar to sunlight. But in more recent years, antibiotic-resistant strains of diseases such as TB are emerging, proving that medicine may no longer be enough.

    Combine the anti-sun skin cancer message and the trend for young people to play indoors on computer games rather than running around outside and there is the potential for a health crisis. In 2009 we are simply not getting enough sunlight to make the Vitamin D we need to stay healthy. Dietary sources of Vitamin D include oily fish, liver, eggs and fortified foods such as cereals, margarine and powdered milk. There are supplements, such as cod liver oil, but it’s almost impossible to get sufficient Vitamin D from diet alone. A person would have to drink ten tall glasses of Vitamin D-fortified milk each day just to get the minimum levels.

    There are several pills available in low doses (the European Recommended Daily Allowance is 5mcg). But, despite being a doctor, I am not a great fan of pill-taking, especially as there is controversy over what is a ‘safe’ level of the vitamin. In the early Fifties, use of Vitamin D supplements in food caused infants to die of heart and kidney failure. But, by contrast, it is impossible to overdose through sunlight exposure. The skin automatically switches to manufacturing pigment (resulting in a tan) when it has made enough Vitamin D. Sunshine really is the best source. This doesn’t mean you should throw yourself at a beach once a year and let yourself burn. The skin cancer message is right. Melanomas – tumours on the skin – are found on people who have burnt in the sun, particularly when young.

    Best estimates from a recent World Health Organisation report are that, on a sunny day, a fair-skinned person will achieve maximum Vitamin D production from exposing face and forearms to midday sunlight for five to ten minutes, up to three times a week. Short exposure gives the benefit and long exposure causes harm. Of course, in this country, sun can be hard to come by, especially during the winter. It’s during these times that many of us may benefit from the UV lamps used in sunbeds. Because the whole body is usually exposed, a single five to ten-minute session once every six weeks should be sufficient.

    Those with darker skin may need up to eight times as much exposure as someone of a Caucasian background because their skin is more resistant to UV rays. The danger with sunbeds comes if they are used by children. The risk of melanoma increases for those who start using sunbeds under the age of 18. But a recent study of European melanoma rates found that in Sweden – where sunbed use is strictly regulated and tanning salons are supervised, preventing children from using them and protecting adults from over-exposure - there was less melanoma in sunbed users than in non-users. I doubt GPs will ever prescribe a short sunbed course to someone with a Vitamin D deficiency, but I don’t think there would be any harm if they did. Every little helps.

    Tanning, whether by sunlight or sunbed, may cause pre-mature aging of the skin and skin cancer. Exposure to UV-B, whether by sunlight or by sunbed, produces Vitamin D in the body. However, mild exposure to UV-B is sufficient. Tanning is not required to generate Vitamin D. Vitamin D levels in the body may be maintained or increased by oral supplements without tanning. Increased Vitamin D levels have been associated in some studies with a reduced risk of such conditions as breast, colon, prostate and ovarian cancers, multiple sclerosis and diabetes.     Protect Your Breasts With Vitamin D

    There's a paradigm shift going on in medicine as new research reveals a far greater role for vitamin D. Vitamin D is not just for kids -- or the prevention of rickets. Optimal levels of Vitamin D (40-80 ng/ml) enhance the creation and functioning of healthy cells throughout the body. In addition to protecting the bones and boosting the immune system, studies show that Vitamin D helps prevent certain cancers, including breast, ovarian, prostate, and colorectal.Exciting new research shows that in the U.S. alone, thousands of new cases of breast cancer could be prevented every year if more women had optimal levels of vitamin D.

    A study conducted by Cedric Garland and other prominent vitamin D researchers determined that women with vitamin D levels above 52 ng/ml have half the risk of developing breast cancer as those with 13 ng/ml! Garland (et al) estimates that 58,000 new cases of breast cancer in the U.S. could be prevented per year by raising vitamin D levels to 52 ng/ml. Imagine what the global impact could be!

    A simple blood test is all that's needed to find out your vitamin D level. Five years ago, a range of 20-100 ng/ml was considered normal. Just recently, this range was raised to 32-100 ng/ml. Make sure to ask your healthcare provider what your actual vitamin D level is. Too often women are told that their levels are normal, which is not the same as optimal.

    If you're deficient, the best way to boost your vitamin D quickly is to supplement with vitamin D-3. Initially, you may need to take 5,000 IUs per day. After establishing a healthy level, I recommend supplementing with1,000-2,000 IUs per day--it's hard to get all you need from food. Some healthy fish provides 300-700 IUs, but milk only provides 100 IUs per glass.

    You may be surprised to learn that the sun is actually the best source of vitamin D. The sun's UVB rays enable our bodies to manufacture vitamin D in the fat layer under the skin, as long as we don't use sunscreen. The body can make enough vitamin D from sun exposure to last the entire year! And it will never create toxic levels, regardless of how long you expose your skin. Although we are taught to fear the sun, sunbathing in moderation -- exposing but never burning the skin -- is good for us. This may explain why the incidence of breast cancer is higher in northern latitudes than at the equator.

    I encourage every woman to check her vitamin D level regularly and keep it in the optimal range. This is easily done by supplementing with about 2,000 IUs of vitamin D-3 per day and getting regular, safe sun exposure. (You can even visit a tanning salon that offers UVB tanning rays.) Your breasts and your entire body will benefit. This is preventive medicine at its finest.
